In 2015, we partnered with the Allegheny County Department of Human Services to provide health coverage enrollment assistance to consumers upon release from incarceration at the Allegheny County Jail. We began with one full-time Certified Navigator. In that first year, we completed 274 Medical Assistance enrollments.
Now in our third year, we have expanded our Navigator staff at ACJ and worked with the County to improve systems and offer more services. Our Navigators now complete about 100 Medical Assistance enrollments per month! We focus on enrollment for consumers with mental and behavioral health conditions, many of whom are being released to a treatment program.
Our ACJ Navigation team is now certified to complete SOAR applications. The SSI/SSDI Outreach, Access, and Recovery (SOAR) program assists eligible consumers who are homeless or at risk for homelessness and have a serious mental illness, medical impairment, and/or co-occurring substance use disorder. By providing case management assistance to submit a complete application for benefits, consumers are more likely to be approved for SSI/SSDI. Our team recently submitted its first two SOAR applications to the Social Security office.
