Community Health Workers
Community Health Workers (CHW’s) are members of a community, trusted and trained to be a
“go-to” when fellow community members have questions regarding their own and the
community’s health and wellness.
Our Community Health Workers have assisted with:
Social Determinants of Health:
o Transportation
o Food
o Environment
o Air quality
o Access
Health and Wellness:
o Making the most from every medical encounter
o Being an ACTIVATED patient
o Diabetes management
o Smoking Cessation
o Understanding what tests and why
Currently, we have CHW’s working in Clairton and with the Bhutanese refugees in the South Hills
